Instructions.  Click on the document in the right column to download and complete the form.  Note, if you are completing the registration paperwork by phone, we encourage you to click on the forwarding icon in the top search bar after you have opened the form and send the form to your Google browser.  From there, when you open it, Google will give you the choice on your mobile device to open in the Google Docs app which will allow for editing the document directly on your phone or tablet.  If you do not have the Google Docs app, Google will prompt you to download it in either the App Store (iPhone) or Google Play Store (Android).
